My Story

My Decision

I want to start by saying that I almost did not submit my story because it is not a fairy tale. It is a good outcome, not a great one, and I think that is actually more useful to most people than another glowing five-star review. Real stories have nuance, and mine has nuance. I am 56. I have watched my jawline disappear over the past decade. Part of it is aging, part of it is genetics — my mother looked the same way at this age. I had gained about 12 pounds since menopause and most of it seemed to settle in my neck and face. Even after losing the weight, the fullness under my chin and along my jawline persisted. I considered a full lower facelift. My dermatologist actually recommended one. But the cost (north of $20,000), the recovery time (4-6 weeks), and the general anesthesia requirement all gave me pause. I live alone, I work full-time, and I do not have someone to take care of me for a month. I started researching whether liposuction alone could make a meaningful difference. What I found is that liposuction works well for the fat component of neck fullness but does not address loose skin or platysmal banding. At 56, skin elasticity is a real factor. I went in knowing that liposuction would not give me the jawline of a 35-year-old. The question was whether it could give me a noticeable improvement. The answer is yes — with caveats.

Finding the Right Surgeon

I consulted with three surgeons because I wanted honest assessments from multiple perspectives about what liposuction alone could do for a 56-year-old neck. Surgeon one recommended a lower facelift and was borderline dismissive when I said I was considering lipo only. He said I would be disappointed with a half-measure. Maybe he is right from a purely technical standpoint, but his bedside manner eliminated him immediately. Surgeon two was willing to do lipo only but gave me what felt like an overly optimistic projection. He said my skin would retract beautifully. At 56, with visible skin laxity, that felt like he was telling me what I wanted to hear rather than the truth. Dr. Liu was the Goldilocks consultation. She examined my neck thoroughly — pulling the skin, measuring the fat layer with her fingers, checking the skin elasticity by pinching and releasing. She said: "I can remove the fat and give you a cleaner neck contour. Your skin will retract some, but not completely. You will have improvement — meaningful improvement — but you will still have some skin laxity that only a lift can address. The question is whether that improvement is worth it to you." That was the most honest thing any of them said. I said yes.

The Consultation

Dr. Liu estimated about 30-40 cc of fat removal from the submental area and along the jawline. She planned to use microcannula technique through two incisions — one under the chin and one behind each earlobe. The incisions behind the earlobes would let her access the jowl area and the lateral neck. She recommended IV sedation rather than local-only because the jowl work involves areas closer to nerves, and sedation would keep me comfortable and still during a procedure she estimated at about 75 minutes. She showed me before-and-after photos of three women in their 50s who had the same procedure. The improvement was real but not transformative. The after photos showed cleaner jawlines and reduced double chins, but you could still see some skin laxity. She pointed this out proactively. One of the patients had subsequently gotten a mini facelift a year later and the combination was excellent. She mentioned this as an option, not a sales pitch. She also told me that the neck takes longer to show final results than most areas because the skin is thin and swelling in the neck can persist for 2-3 months.

Procedure Day

I went in on a Friday morning. The IV sedation was pleasant — I felt relaxed and vaguely aware that something was happening, but I was not anxious about it. I could hear Dr. Liu talking to her nurse but could not tell you what they said. The procedure itself was painless under the sedation and local anesthetic combination. I went home wearing the chin strap compression garment. I looked in the mirror and saw a swollen woman with a chin strap and thought, well, no going back now. My neighbor had agreed to check on me and bring dinner. She took one look at me and said, "You look like you went ten rounds." Fair enough.

Recovery Experience

The recovery was easier than I expected in terms of pain — I never needed anything stronger than Tylenol — but harder than I expected in terms of patience. The swelling was moderate for the first week. The bruising was mostly under my chin and along my jawline, hidden by the chin strap during the day. I went back to work on Monday — three days post-op — with the chin strap under a turtleneck. One colleague asked if I had a neck injury. I said I had slept wrong on it. That satisfied her. I wore the chin strap for three weeks: 24/7 the first week, daytime plus sleeping the second week, sleeping only the third week. The frustrating part was the gradual nature of the reveal. At week two, I honestly was not sure I could see any difference. At week four, I thought maybe there was some improvement. At week eight, I looked in the mirror and definitively saw a cleaner, more defined jawline. My sister visited at month two and said, "You look great — have you lost weight?" That was when I knew it was working.

Results

Six months post-op, here is my honest assessment. What improved significantly: the double chin is substantially reduced. The front-on view of my face looks notably different — cleaner, more defined, less heavy. The jowl area is improved — not eliminated, but reduced. Profile view is better. I look like a younger, fresher version of myself, not like a different person. What did not change: I still have some skin laxity along the jawline. When I look down at my phone, I can see the skin bunch slightly under my chin. The platysmal bands are unchanged — liposuction cannot address those. In harsh overhead lighting, the skin texture is not as smooth as it would be after a facelift. The honest summary: if a facelift would have given me an A, liposuction gave me a B or B-plus. That is not a criticism — I knew going in that a B was the ceiling. For $4,800, three days of downtime, and no general anesthesia, a solid B-plus feels like a good deal. I am rating this 4 out of 5 stars — not because I am dissatisfied, but because I want to be accurately helpful. If you are in your fifties considering neck lipo, go in with realistic expectations and you will be pleased. Will I get a mini facelift down the road? Maybe. The lipo gave me an immediate improvement that buys me time to decide.

My Tips for Others

  • Get multiple opinions and listen to the most honest one — I consulted three surgeons. The one who told me the most realistic truth got my business, and I am glad
  • Skin elasticity matters a lot after 50 — the fat removal component of lipo works the same at any age. The skin retraction does not. Understand this distinction before you go in
  • Neck lipo recovery is easier than most body areas — I was back to work in three days. The chin strap is the main inconvenience, and it is manageable with turtlenecks or scarves
  • Give it three months before judging — neck results take longer to appear than you expect. I almost panicked at week two. By month three, I was pleased
  • Lipo first, facelift later is a valid strategy — if you are unsure about a facelift, neck lipo at a fraction of the cost can give you meaningful improvement. You can always add a lift later, and having the fat already gone makes the lift procedure simpler
  • Living alone during recovery is manageable for neck-only lipo — I cooked, cleaned, and managed everything myself by day two. Harder areas like abdomen or thighs would be different
